Referrals through this pathway are for individuals who are not currently in receipt of funded Adult Social Care, but whose independence is at risk due to their diagnosis and are more likely to begin needing funded support within 6-24 months. Funded social care related to their cognitive needs. Risk is either immediate or likely to be apparent in the near future.

A person's suitability for the pathway will largely be left to the discretion of the referrer, provided that they fall within the eligibility criteria. This approach reflects the fact that appropriateness for referral depends on a range of clinical and non-clinical factors

Both of the following are requirements for eligibility:​

The person is living in Leicestershire and has capacity to consent to both the referral and install of Care Technology

1.The person has either:

a) A clinical diagnosis of dementia of any kind ​
 
b) A clinical diagnosis of Mild Cognitive Impairment (MCI)​
 
c) Exhibiting symptoms of dementia and awaiting professional assessment

If the person meets the requirement of 1a or 1b, 1c and either 2a or 2b then:​
 
2. One of the following is an additional requirement:

a) The person's clinician, Dementia Adviser, Adult Social Care Professional are becoming concerned about their ability to manage independently at home and thinks that they would benefit from Care Technology.
 
b) The person is in hospital, but the hospital discharge team is concerned about a safe discharge. The person could be discharged from hospital more confidently with Care Technology. ​
