• Completing this online self assessment is the fastest and easiest way to find out what care and support is available to you, and from where it can best be obtained.
• It will take between 20 to 30 minutes to complete this assessment.
• If you qualify for support from us, you will have to complete a financial assessment. This will tell you how much you will have to pay towards your support.
• You can save the form and return to it later by completing our simple registration process; all you will need is your name, address, and email address.
• Please do not use this form to report a safeguarding incident.

If your needs are not as a result of mental or physical difficulties, it is unlikely that you will be eligible for support from Leicestershire County Council at this time.
