PBS Referral Process
The PBS service will support LCC funded cases only whether that be fully or joint with the CCG. There are three primary roles for the PBS service within LCC
• To support individuals, and the provider to maintain their current placement or support setting and prevent placement breakdown (in line with TC guidance).
• To support the review process, where required, of people in existing support settings and packages as part of a progression approach to increased independence.
• To support and upskill providers in PBS processes and techniques so they can support, maintain and progress people with learning disabilities, and/or autism in the community.

The PBS service predominantly supports adults (18+) but will work with CYP services on transitions cases (16+) where possible. It is expected that the service will work with people who have; Dementia, LD and Autism
The PBS service is not a crisis service: that being the role of the Outreach Team provided by LPT and commissioned by the CCGs across LLR. The PBS service will operate at a less intensive level, so people at imminent risk of admission to hospital and therefore subject to a ‘blue light CTR' fall outside of the criteria. Instead, it is hoped that the PBS service will intervene at an earlier stage and so affect the number of people requiring the Outreach service for crisis interventions.

Once agreed that it is an appropriate referral it will be allocated to a PBS worker
