This form is solely for the use of Community Therapists and Vista to request Adult Social Care OT authorisation for the following minor adaptation requests: Concrete half steps, external metal rails, drop down rails beside a toilet, floor to ceiling poles and wall fixed drop down shower seats. Please note: You will need to attach a minor adaptation form, a supplementary information form and a diagram with measurements to this request.
